  Transitional Living Program (TLP)

Community Care Services
The Transitional Living Program is a closed referral program funded by the Department of Children and Families. TLP is a residential service for young women and their children which includes: individual and group counseling; individualized case management services; independent living skills; parenting skills assessment and education; educational and job training services; medical, dental, and day care referrals; recreational programming, housing search and aftercare support. Through these services and community resources, this program offers clients an opportunity to become self-sufficient.

In 2008 fifteen teen moms were served in our program where six healthy babies were born.
