Journey Home is a newly developed group home
program of Community Care Services. Like its
name, the program is designed to assist youth as
they begin the journey of hope and discovery and
navigate through the obstacles of life to find a way
home. Its mission is to serve adolescents who
present with multiple issues and behaviors and
transition them into stable foster homes as quickly as possible.

Since its inception in May 2008 Journey Home has worked with 42 service recipients and their families. The Journey Home Program and our Agency's Intensive Foster Care Program work collaboratively;of the 42 service recipients, we were able to place 25 young men in foster homes when other agencies were unable to find a match. In addition, Journey Home has helped seven families reunify and continues to ensure that each young man finds his "journey home."
